Sometimes patients have complex needs when they leave the hospital. They may need the services of many different professionals, agencies, healthcare facilities, and human services programs. Professionals in case management are available to help coordinate and manage all these services.

Social Services provides a variety of counseling, support, and referral for situations such as terminal illness, drug and alcohol rehabilitation, crisis intervention, paternity issues, cancer programs, and medical ethics.
