Cardiac Rehabilitation
If you’ve had a heart attack, heart surgery or if you have a high risk
for cardiac problems, you may be a candidate for the Cardiac
Rehabilitation program at Memorial Hospital. This medically supervised
program of exercise and education is designed in cooperation with your
doctor to reduce your risk of future heart disease. Cardiac
rehabilitation will help you:
- Establish an effective, personalized exercise program
- Modify your risk factors, including cholesterol levels
- Recognize the signs and symptoms of heart disease
- Know when to seek
medical help
- Understand how good nutrition enhances your cardiac health
- Prepare to return to work and other activities
While in cardiac rehabilitation, you will attend heart-health education
programs. You will also exercise while wearing a heart monitor three times a
week for up to 12 weeks. Your personal exercise program will be
modified to suit your age, health and physical ability. Once you
complete cardiac rehabilitation, Memorial Hospital offers an adult
fitness program to help you continue your heart-healthy lifestyle.

Availability:
The Health Center is open from 7:00 a.m. to 7:00 p.m. on Monday through Thursday and on Fridays from 7:00 a.m. to 4:30 p.m.
Appointments:
No matter where your physician is based, we welcome your participation
in our Cardiac Rehabilitation program. The Health Center staff will
call you to schedule your first appointment and verify your insurance
coverage.
Enrollment:
To enroll in cardiac rehabilitation, a referral from your doctor is
required. Once enrolled, our Health Center staff works closely with you
and your doctor to meet your individual program objectives and provide
regular reports on your progress.
Transportation:

Registration:
Registration is completed in the Health Center office. Please bring
your insurance card, driver's license or state ID, and written physician order to your first
appointment.
