National obesity rates are at an all-time high, even for children. More
and more people are diagnosed with adult onset (Type II) diabetes. High
cholesterol and body fat are major culprits in heart disease. Memorial
Hospital wants to put you back on the road to healthy eating by
offering ready access to complete Nutrition Services.
We’ll consult with you to determine your body’s nutritional and
therapeutic needs and develop a dietary program that accomplishes your
goals. Whether it’s weight loss, weight gain, sports nutrition or the
nutritional management of medical conditions like diabetes or heart
disease, you’ll find the knowledge and support you need from our
registered dietitians and staff. Nutrition Services are also available
to patients in the hospital to ensure that any special dietary
requirements are met and that the patient is equipped with proper
nutritional guidance to continue a healthy recovery at home.

Location:
Just inside the Morey Center entrance off London Avenue, adjacent to the cafeteria.
Availability:
Office hours are Monday through Friday, 9:00 a.m. to 5:30 p.m.
Appointments:
Appointments can be made by your physician's office or yourself. You
may want to verify with your insurance carrier that medical nutrition
therapy is or is not a covered service under your plan.
Requirements:
If you would like to have your insurance carrier billed for the medical
nutrition therapy consult, a physician order must be received by the
time of your appointment. The physician order must include diagnosis,
request for dietitian consult and your physician's signature. Your
physician's office can fax a referral directly to our office at (937)
578-2371.
If you would like to self-pay for your appointment, a physician order is not necessary.
Registration:
We will register you in the Food and Nutrition Services office. Please
plan to arrive 15 minutes early for your scheduled appointment to
complete necessary paperwork.
