Our Social Services and Case Management team partners with your
healthcare providers to carefully plan what services you might need
both during your stay and after you are discharged from the hospital.
We begin this planning when you are admitted. Our case managers contact
your insurance company for benefit information and pre-certification
for services needed during and beyond your hospital stay. Our staff is
involved in your care and discharge planning, helping connect you with
the resources you need when you go home. We can assist you with:
- Assisted living or extended care facilities
- Home health care agencies
- IV/TPN services
- Medical equipment providers
- Support groups
- Rehabilitation programs
- Hospice services
- Cancer treatment
- Social security
- Emergency response programs
- Counseling services

- Assistance for crime victims
