When illness or injury strikes, you’re left with all kinds of questions and decisions to make. Getting the best medical care is priority one, but what happens when you go home? Will you need extra medical care? Will you be able to care for your home, children or pets? Who will drive you to your doctor’s office? These are just a few of the worries that can hinder the progress of a healthy recovery. Memorial Hospital is here to support you by linking you to the community resources and programs you need to get back on your feet.
Our Social Services and Case Management team partners with your healthcare providers to carefully plan what services you might need both during your stay and after you are discharged from the hospital. We begin this planning when you are admitted. Our case managers contact your insurance company for benefit information and pre-certification for services needed during and beyond your hospital stay. Our staff is involved in your care and discharge planning, helping connect you with the resources you need when you go home. We can assist you with:
- Assisted living or extended care facilities
- Home health care agencies
- IV/TPN services
- Medical equipment providers
- Support groups
- Rehabilitation programs
- Hospice services
- Cancer treatment
- Social security
- Emergency response programs
